You will need to bring the following documents to your appointment. If you do not have all the required documents completed and in hand at the time of your scheduled appointment time, you will be rescheduled for a later date:
- Packet of documents completed and legible. Find Documents HERE
- Photo ID
- Proof of financial hardship
- Read ALL information below ↓
IF APPLYING FOR RENT ASSISTANCE, you will need to provide the following in addition to as stated above:
- Copy of CURRENT lease
- Ledger print out from Property Manager
- If you rent from an individual who does not have a ledger, we will need copies of receipts from the last three consecutive payments made
- This Landlord Statement, completed by Landlord (form found HERE)
IF APPLYING FOR UTILITY ASSISTANCE, you will need to provide the following in addition to as stated above:
- Utility bills (if you owe multiple months, and you do not have the past due bills, have your utility company provide you the necessary back statements)
